(15) Blocks 16 through 19. They are to be used for OX transactions as follows:
a. Block 16. The repairing facility will enter the job order number,
b. Block 17. The individual receiving the item at the repairing facility will enter his
initials.
c. Blocks 18 and 19. The individual accomplishing-- the repair will enter the date and his
.
initials in blocks 18 and 19, respectively.

Section VI. ADDITIONAL INFORMATION
CONTESTED CLAIMS. If, upon analysis of the failed part(s), it is determined by
Cummins/Detroit Diesel Allison or component suppliers that failure was not due to defective
workmanship or material, the Government will be invoiced for the list price of the part(s), labor
borne by Cummins/Detroit Diesel Allison in the repair, and related freight costs.
